2024-2025 Enrollment Forms
Links to download the 2024-2025 Enrollment Form and Permission Form is below (look for the pdf file symbol below). These forms should be turned in to Gateway Fairport as soon as possible. If you are a current family, you may bring in your forms and place them in the tuition box or hand them to me at drop off or pick up.
If you are new family, you can either bring the forms in during school hours M-F 9:00 -11:45 am and T-Th 9:00 am – 3:15 pm or you may choose to send them back to us (Gateway Fairport Nursery, 31 West Church Street, Fairport, NY 14450)

Covid Policy
COVID Policy is listed on page 7 if the handbook. This is the latest COVID Guidelines as per the May 31, 2022 by the Monroe County Department of Health Guidelines. Please note that the COVID policy may change at any time based on local, state or federal requirements.
Sick Policy
Children must stay home if they are sick. In order for your child to return to school they must be symptom free and medication free for AT LEAST 24 hours. If your child has seasonal allergies, please bring in a doctor’s note for the child’s file. As we all know COVID symptoms are very similar to seasonal allergy symptoms.
